Q CODES & CPT CODES
Each telepsychiatry session generates two billable codes:
- The standard CPT codes for outpatient, psychiatric services.
- An originating site fee known as Q3014. The originating site fee is a Telemedicine Bonus that allows clinics to receive reimbursement for the use of the facility where telehealth services are rendered.
GETTING STARTED
Who can be a telepsychiatrist for my clinic?
Psychiatrists: Any psychiatrist (MD/DO) with a state license in Texas can provide care via telepsychiatry.
APRNs: APRNs require collaborating physicians in order to practice telepsychiatry in Texas with monthly meetings to review treatment plans and charts. A recently passed law removed the face-to-face requirements for APRNs practicing telepsychiatry, allowing APRNs to practice telepsychiatry more freely in Texas.
How does reimbursement work for telepsychiatry?

Medicare
Community mental health clinics are eligible for Medicare reimbursement in Texas if they are located in a HPSA (Health Professional Shortage Area). You can check if your address is eligible for Medicare telehealth originating site payment of here.

Medicaid
In Texas, clinics can bill the appropriate CPT code AND the Q code of $18.27 per child and adolescent patient encounter and $17.40 for adult patient encounters.

FQHC
FQHC telemedicine providers must submit their claims using the appropriate encounter code and modifiers. Use modifier AM or SA in the first modifier field on the claim form with the modifier GT in the second modifier field.
What tech does my clinic need?

Setting up telepsychiatry at your clinic is simple. The set up will require:
- A large monitor screen
- A two-way video telecommunication system that allows for real-time communication between the provider and patient
- HIPAA-compliant video conferencing technology
UTILIZATION & PROGRAM SUSTAINABILITY
